Google is testing healthcare ads in AI Mode, but ads that use pinned assets or text disclaimers are excluded.
Google is testing healthcare ads in AI Mode, and any ad that uses pinned assets or text disclaimers is excluded, per Marsad Group. For a certified rehab account, that one rule matters more than the test itself, because pins and disclaimers are the tools compliance-conscious teams use to lock wording in place. Sweet Media, founded in 2023, runs LegitScript-certified Google Ads accounts for behavioral health, and this post lays out what the trade-off looks like from inside that work.
Google confirmed a limited test of healthcare ads in AI Mode, restricted to U.S. advertisers and English-language queries, according to DesignRush. PPC Land reported it around Google Marketing Live on May 20, 2026, and Search Engine Roundtable relayed comments from Google Ads Liaison Ginny Marvin in early June.
This is a policy shift for a category Google had fully excluded from that surface since AI Mode launched, per Marsad Group. It is still a small test and not a rollout, so many rehab accounts may see no impact yet, and we won't guess at when or whether it expands.
Google told Adweek it is testing ads for certain pharmaceutical products in AI Mode and AI Overviews.
The test includes prescription drugs, began this month, and is limited to the United States.
Pharmaceutical advertisers can run standard text and Shopping ads in sections labeled "Sponsored," according to Adweek.
Google's statement says it "recently expanded" its AI Mode and AI Overviews ad test "to include certain pharmaceutical products."
Search Engine Roundtable reports that Google said there were no recent policy changes behind this test.
Barry Schwartz of Search Engine Roundtable also wrote that he searched and did not spot any of these ads himself.
Neither source mentions addiction treatment, telehealth, or medication-assisted treatment in connection with this test.
Still, a sponsored drug ad could appear in the same AI answer where a telehealth or MAT provider appears.
That possibility is our inference from the test's scope, not something Google or either outlet has stated.
Google's Healthcare and medicines policy still requires LegitScript certification to advertise drug and alcohol addiction services.
Because Google reported no policy change, we see no sign that addiction-services certification rules have changed.

Keep ad copy and landing pages free of patient health details, and keep tracking setups aligned with HIPAA.
We will update this section if Google publishes new policy language or if these ads appear in results.
A standard search ad sits next to a results page you can predict by keyword. In AI Mode, the ad appears alongside a generated answer that the advertiser neither writes nor controls, so your ad may sit beside an explanation of treatment options whose claims you cannot shape.
Search Engine Land reports that the same campaign types are also eligible to serve ads in AI Overviews, so the same creative limits apply in both places.
Performance Max, AI Max with search term matching, Shopping and broad match campaigns are eligible, per DesignRush. Shopping has no role for a treatment center, so in practice the choice is among the other three. Ads that use pinned assets or text disclaimers are excluded, per Marsad Group, and nothing in the sources suggests other campaign types qualify, so check your account's mix against that list before assuming anything.
Account setup matters here, because a tightly controlled exact and phrase match structure is built for the opposite of what these formats reward. Google's own help page says it requires AI-powered targeting such as broad match or AI Max for Search campaigns on these surfaces (Google Ads Help), and Marsad Group lists exact and phrase match-only campaigns as not mentioned as eligible, so treat them as untested (Marsad Group).
Google's help page also says advertisers can't directly target or opt out of ad placements in AI Overviews (Google Ads Help). It doesn't address this healthcare test specifically, so the practical step is to ask whoever manages your account which campaign types are running.
Eligibility requires giving up pinned headlines and text disclaimers, and for a certified rehab account that is the whole trade-off. Marsad Group notes that campaigns with tightly pinned headlines or disclaimer lines simply won't show in AI Mode under the current test. Pinning guarantees a specific line, such as a certification statement or a careful description of services, appears in a fixed position, and without it the system assembles headlines on its own.
The sources describe the test as healthcare advertisers in the U.S. and don't say whether certified addiction-treatment advertisers are eligible (DesignRush). We can't tell you your account qualifies, and no one outside Google can confirm it from the public record.
There is also a compliance gap to weigh. DesignRush says the first phase sidelines most compliance-heavy healthcare creatives (DesignRush), so missing AI Mode traffic may not be a performance problem. It may simply mean your locked wording keeps you out, and that wording may be worth more to you than the extra surface.

Google's ad format doesn't make your lead handling compliant or non-compliant, since your tracking and form setup does that. Marsad Group advises verifying that server-side, consented tracking already works on standard Search and Display before opting into AI Mode placements (Marsad Group). Any conversion measurement that passes caller or patient details to an ad platform needs privacy review before you rely on it.
Preparation costs little and pays off whether or not you ever see AI Mode impressions. Start with how you would measure it, because you can't judge a new surface you can't separate from the rest, and Marsad Group makes the same point about checking tracking before opting in, not after (Marsad Group).
DesignRush calls for incrementality testing and clean conversion tracking, and says you cannot build that baseline retroactively (DesignRush). Conversion tracking has to tie to admissions and not just form fills. Track calls, verification of benefits requests, and admits, then compare against a holdout so you know whether AI Mode adds admissions or relabels ones you would have gotten anyway. Keep that tracking free of identifiable health information.
Marsad Group argues that a landing page which answers a specific question directly, with real credentials visible near the top, will outperform a generic service page built for keyword-matched traffic (Marsad Group). Service pages that describe each level of care in plain language also give automated formats less room to guess about what you treat and who you serve.
Check four things before you change anything. First, confirm whether you run Performance Max, AI Max, or broad match today. Second, confirm your legal reviewer can approve copy that carries no pinned line or text disclaimer, since that is the condition Marsad Group describes for showing in AI Mode. Third, make sure you track admits as well as leads, and fourth, make sure you can isolate this traffic from standard Search.
If any of those checks fails, fix it first. A rehab team that can't clear the legal review step should stay out, because compliance wording is the asset you'd be trading away.
| Choice | What you gain | What you give up |
|---|---|---|
| Strip pins and disclaimers | Possible access to AI Mode placements | Fixed wording and in-ad compliance language |
| Keep pins and disclaimers | Locked, reviewable messaging | Exclusion from this test |
| Split eligible and non-eligible ad groups | Protection for sensitive messaging | Extra account complexity to manage |
The exclusion of pinned and disclaimer ads comes from Marsad Group and DesignRush. Splitting is our own suggestion for regulated advertisers. Keep compliance-sensitive messaging in locked ad groups and put any rewritten, eligible copy in its own group, with legal or compliance reviewing every rewrite before launch.
Sweet Media is a behavioral health digital marketing agency in Costa Mesa, California, and we treat paid media as admissions infrastructure. Campaign structure, tracking, and compliance wording get decided together, not in separate meetings. Our paid media work covers the account structure and measurement this test would demand.
We manage LegitScript-certified rehab accounts, so we see the pinning and disclaimer trade-off in live accounts and not in theory. Our guide to the Google Ads healthcare and medicines policy covers the baseline rules, and our drug rehab PPC post covers campaign structure.
Performance Max and AI Max pull budget and signals across surfaces, and Search Engine Land lists both among the eligible campaign types. Adding AI Mode eligibility to one campaign can shift spend away from the keyword-controlled campaigns your admissions team trusts, so watch search term reports closely and keep negative keywords tight.
Eligibility for certified addiction-treatment advertisers has not been spelled out. The public reporting describes a limited U.S. test for healthcare advertisers (DesignRush), and the sources don't confirm that certification qualifies an account.
You lose control over which headline appears and in what position, so if a certification statement or careful service description depends on a pin, the system may drop or reorder it. Ads that rely on pins are excluded from the test entirely (Marsad Group).
Text disclaimers exclude an ad from the test (Marsad Group). DesignRush suggests building required claims and context into the creative itself (DesignRush), but have your compliance reviewer approve any such wording rather than assuming it works.
Google has not documented an opt-in or a checkbox for this test, and its help page says advertisers can't directly target or opt out of AI Overviews placements (Google Ads Help). Ask your agency or internal team which campaign types and match types are running, and compare them to the eligible list.
The sources describe healthcare ads broadly and don't break out mental health or substance use. Treat both as unconfirmed until Google says otherwise.
Use a holdout or incrementality test that tracks admits as well as leads, and keep conversion data free of identifiable health details. DesignRush stresses that this baseline can't be built retroactively.
